Atuação do farmacêutico residente no ambulatório de transplante de medula óssea: relato de experiência
Abstract
O transplante de células-tronco hematopoéticas (TCTH) representa o tratamento de
escolha para um grande número de doenças hematológicas, neoplásicas,
congênitas e hereditárias. O TCTH tem como objetivo substituir a medula óssea
doente mediante o procedimento de infusão de células tronco CD34, que podem ser
do próprio paciente (autólogo) ou de doador (alogênico aparentado ou não
aparentado) e que vai reconstituir uma medula saudável. Este estudo objetiva
descrever a atuação do farmacêutico residente no ambulatório de transplante de
células-tronco hematopoéticas no pré e pós-transplante dos pacientes com doenças
onco-hematológicas, realizado em um hospital do Estado do Rio Grande do Sul.
Trata-se do relato de experiências vivenciados nas duas fases do atendimento
ambulatorial: Pré-transplante – período no qual o paciente recebe acompanhamento
ambulatorial até a internação, com orientações sobre as etapas do procedimento,
como: sua mobilização, a qual tem objetivo de aumentar a quantidade de célulastronco para a coleta; a internação para o condicionamento, a qual compreende a
administração de altas doses de quimioterápicos e/ou irradiação corporal total para
erradicar a doença de base, promover imunossupressão e criar espaço na medula
para receber as células-tronco provenientes do doador; a infusão das células e a alta
hospitalar; e o Pós-transplante – inicia-se após a alta, subdividindo-se em pósimediato, até 100 dias da infusão das células, e tardio, a partir desse marco. O TCTH
é uma terapia complexa cujo sucesso envolve diretamente a atuação da uma equipe
multidisciplinar com o trabalho integrado. Neste cenário, está inserido o
farmacêutico, o qual contribui nas diversas etapas, como o manejo de
medicamentos, que inclui conhecimento em terapia antineoplásica de altas doses e
de doenças infecciosas; aconselhamento no uso correto dos medicamentos; manejo
de sintomas, e acompanhamento clínico para efetividade, qualidade e segurança do
paciente transplantado.
