Reanimação cardiopulmonar transanestésica em potro - relato de caso
Abstract
Procedimentos cirúrgicos em equinos neonatos são arriscados pela imaturidade dos seus sistemas orgânicos e processos de homeostase. Entretanto, quando se fazem necessários, é preciso preparação da equipe para sucesso no tratamento de intercorrências transanestésicas. O presente trabalho tem por objetivo relatar o caso de sucesso na reanimação cardiopulmonar e a conduta anestésica em uma potra de 30 dias de idade submetida a cirurgia de osteossíntese em metatarso bilateral. A paciente em questão iniciou o procedimento com hipotensão severa, e após 15 minutos sob anestesia geral, apresentou atividade elétrica sem pulso e ausência de reflexo corneal, quando imediatamente foram iniciadas as manobras de suporte básico à vida, com compressões torácicas e ventilação com FiO₂ de 100%. Em seguida teve início o suporte avançado à vida, utilizando adrenalina intravenosa (IV) em dose de 0,01mg/kg por dois ciclos e dose de 0,1mg/kg IV no último ciclo realizado, onde se obteve êxito na reanimação. Após retorno espontâneo à circulação, foi dado seguimento ao procedimento cirúrgico. Ao recuperar a consciência, despertou com sinais de lesão hipóxico-isquêmica cerebral, além de não conseguir adotar posição quadrupedal ou esternal. Por fim, o equino retornou ao internamento e deu seguimento ao tratamento, entretanto, a paciente veio a óbito após 7 dias da parada cardíaca.
