dc.description.abstract | A hyperinflation caused by chronic obstructive pulmonary disease (COPD) promotes diaphragmatic rectification, reducing a zone of apposition of the diaphragm and length of muscle fiber, resulting in loss of muscle contraction, respiratory muscle strength. Chronic lung diseases tend to be non-specific, as does compensation to maintain a respiratory function. However, this mechanism reduces airway protection, facilitating pulmonary penetration or aspiration. The present study aimed to relate the biomechanics of swallowing and the diaphragmatic mobility of subjects with COPD. It is an observational, transversal and descriptive study involving 19 COPD subjects. Body mass index, airflow obstruction, dyspnea and exercise capacity (BODE), diaphragmatic mobility and videofluroscopy of swallowing, body mass index, airflow obstruction, disability and capacity of exercise. The results showed that dysphagic subjects (n = 3) had mild (n = 2) and moderate / severe (n = 1) dysphagia. Subjects who presented null swallowing in a discrete manner did not present diaphragmatic mobility, while the subject with moderate / severe dysphagia presented low diaphragmatic mobility. There was a moderate negative relation between the pharyngeal transit time and diaphragmatic mobility in the net consistency (r: -0.508, p: 0.02). This work contributed with unpublished findings regarding diaphragmatic mobility and risk of deferment. And so, nasíl new researches on the subject with the objective of clarifying this relation better and improvements for the subjects with COPD. | eng |

dc.description.resumo | A hiperinsuflação causada pela doença pulmonar obstrutiva crônica (DPOC) promove a retificação diafragmática, reduzindo a zona de aposição do diafragma e comprimento da fibra muscular, resultando em perda de contração muscular, força muscular respiratória. As doenças pulmonares crônicas tendem a apresentar incordenação entre respiração e deglutição, e alterações funcionais ventilatórias como forma compensatória para manter a função respiratória. Porém esse mecanismo reduz a proteção da via aérea, facilitando a penetração ou aspiração pulmonar. O presente estudo teve como objetivo relacionar a biomecânica da deglutição e a mobilidade diafragmática de sujeitos com DPOC. Trata-se de um estudo observacional, transversal e descritivo, envolvendo 19 sujeitos com DPOC. Os sujeitos foram avaliados quanto a função pulmonar, dispneia, força da musculatura respiratória, impacto da doença na qualidade de vida, índice de Body Mass-Index, Airflow Obstruction, Dyspnea and Exercise Capacity (BODE), mobilidade diafragmática e videofluroscopia da deglutição. Os resultados obtidos demonstraram que sujeitos disfágicos (n=3) apresentaram disfagia discreta (n=2) e moderada/severa (n=1). Os sujeitos que apresentaram alterações na deglutição de forma discreta, não apresentaram alterações na mobilidade diafragmática, enquanto que, o sujeito com disfagia moderada/severa, apresentou baixa mobilidade diafragmática. Houve uma relação moderada negativa entre o tempo de trânsito faríngeo e mobilidade diafragmática na consistência líquida (r: -0,508, p: 0,02). Esse trabalho contribuiu com achados inéditos referente à mobilidade diafragmática e risco de disfagia. E assim, auxiliar novas pesquisas sobre o tema com o objetivo de esclarecer melhor essa relação e melhoras para os sujeitos com DPOC. | por |
