dc.description.abstract | Patients undergoing bone marrow transplantation (BMT) may have oral complications resulting
from stomatotoxicity such as mucositis, xerostomia and fungal or viral infections. Oral
mucositis (OM) is initially characterized by erythema that progresses to ulceration in the
mucous membranes, being one of the most painful complications experienced during this
period. Low-intensity laser therapy (LTBI) is a safe, painless, minimally invasive, inexpensive
and quick-to-apply method, with studies proving its effectiveness in the prophylaxis and
treatment of OM. This study aimed to evaluate the influence of dental care on the
implementation of a follow-up protocol for patients undergoing BMT receiving LTBI. This is
an experimental research carried out at the University Hospital of Santa Maria (HUSM). The
instrument used for data collection was a pre-elaborated questionnaire and the performance of
dental follow-up from the moment of pre-BMT until the patient's discharge. During the BMT
period, the patients were evaluated, on average, 5 times a week, with technical guidance and
monitoring of oral hygiene and maintenance of the mucous membranes with LTBI. 7 patients
were followed, 6 of these were treated only with chemotherapy and only 1 underwent
chemotherapy and total body irradiation (TBI). TLBI was performed in all patients in a
preventive and curative manner. Only 2 patients had OM grade 2 at some time after BMT. No
patient had OM grade ≥3, and two patients did not have OM. It can be concluded that the oral
cavity is affected by the consequences of BMT associated with chemotherapy and/or TBI, in
addition to being a potentially infectious site, of the manifestations, OM was the most frequent,
in the patient who received allogeneic BMT, the development of OM grade 2, with a longer
duration of the lesion in the oral cavity, however only one of the patients in the study underwent
this type of transplant, the elaboration of a protocol pre and post OMT in the follow-up of
patients is essential in the support therapy facilitating the dental routine, TLBI in the prevention
of OM in patients submitted to BMT was effective and this preventive protocol can be instituted
in daily clinical practice collaborating with the quality of life. Therefore, this study showed how
necessary the presence of a dentist is in the multidisciplinary team, in order to collaborate with
the early diagnosis, management and adequate treatment of oral complications. | eng |

dc.description.resumo | Os pacientes submetidos ao transplante de medula óssea (TMO) podem apresentar
complicações orais decorrentes de estomatotoxicidade como mucosite, xerostomia e infecções
fúngicas ou virais. A mucosite oral (MO) é caracterizada inicialmente por eritema com evolução
para ulceração nas mucosas, sendo umas das complicações mais dolorosas vivenciadas nesse
período. A terapia a laser de baixa intensidade (TLBI) é um método seguro, indolor, pouco
invasivo, barato e de aplicação rápida com estudos comprovando a sua eficácia na profilaxia e
tratamento da MO. Esse estudo teve objetivo avaliar a influência dos cuidados odontológicos
para a implementação de um protocolo de acompanhamento de pacientes submetidos ao TMO,
recebendo TLBI. Trata-se de uma pesquisa experimental realizada no Hospital Universitário de
Santa Maria (HUSM). O instrumento utilizado para coleta dos dados foi um questionário préelaborado e a realização do acompanhamento odontológico desde o momento do pré-TMO até
a alta do paciente. Durante o período do TMO os pacientes foram avaliados, em média, 5 vezes
por semana, com orientação técnica e acompanhamento da higienização bucal e manutenção
das mucosas com TLBI. Foram acompanhados 7 pacientes, 6 destes o condicionamento foi
apenas com quimioterapia e apenas 1 realizou quimioterapia e irradiação de corpo total (TBI).
A TLBI foi realizada em todos os pacientes de forma preventiva e curativa. Somente 2 pacientes
apresentaram MO grau 2 em algum momento pós TMO. Nenhum paciente apresentou grau de
MO ≥3, sendo que dois pacientes não apresentaram MO. Pode-se concluir que a cavidade oral
é afetada pelas consequências do TMO associado à quimioterapia e/ou TBI, além de constituir
um sítio potencialmente infeccioso, das manifestações a MO foi a mais incidente, no paciente
que recebeu TMO alogênico foi identificado o desenvolvimento de MO grau 2, com maior
tempo de duração da lesão em cavidade bucal, entretanto somente um dos pacientes do estudo
foi submetido a este tipo de transplante, a elaboração de um protocolo pré e pós TMO no
acompanhamento dos pacientes é essencial na terapia de suporte facilitando a rotina
odontológica, a TLBI na prevenção de MO nos pacientes submetidos ao TMO foi eficaz e este
protocolo preventivo pode ser instituído na prática clínica diária colaborando com a qualidade
de vida. Portanto, esse estudo mostrou o quanto é necessário a presença do cirurgião-dentista
na equipe multidisciplinar, a fim de colaborar com o diagnóstico precoce, manejo e tratamento
adequado de complicações bucais. | por |
