dc.description.abstract | In addition to pulmonary involvement, chronic obstructive pulmonary disease
(COPD) causes different systemic changes, among them muscle dysfunction. Such
alteration can also have repercussions on swallowing since muscle activity during the
transfer of the bolus from the mouth to the esophagus needs to be adequate in order
to maintain airway protection. Thus, the objective of the present study was to analyze
the relationship between handgrip strength (HGS) and pharyngeal transit time (FTT)
in individuals with COPD. This was a quantitative, cross-sectional, and descriptive
study through the analysis of secondary data of the individual database type. The
sample was composed of 16 individuals diagnosed with COPD, of both genders and a
mean age of 65.7 years. Information regarding FPM, FPM asymmetry,
Videofluoroscopy of Swallowing (VFD), Volume-Viscosity Swallow Test (V-VST)
protocol, and Body Mass Index (BMI) were collected from the medical charts. The data
were statistically analyzed using the SPSS (Statistical Package for Social Sciences)
statistical package, version 26.0. Pearson's correlation test, sample effect calculation,
and binary logistic regression were performed. It was observed that most individuals
presented severe COPD (62.5%), reduced FPM, and proportional BMI among the
classifications (malnutrition, eutrophy, obesity). It was also found that most individuals
had normal deglutition (43.75%), V-VST without changes (62.5%), and TTF for liquid
of 0.88 seconds and for pasty of 0.81 seconds. As for the correlations between the
variables, no significant correlation was observed between the swallowing
classification and the rest of the variables. However, a significant correlation (p =
0.012), even if weak (r = -0.608), was obtained between TTF and non-dominant FPM
(FPM-ND). In order to verify the presence of risk for swallowing, the dependent variable
V-VST and the independent variables FPM-D, FPM-ND, and FPM asymmetry were
considered for regression. However, the variables were not significant for the
prediction model. This study showed that individuals with COPD have slower TTF and
weak negative correlation only between the TTF and FPM-ND variables. Furthermore, it was not possible to consider FPM and FPM asymmetry as risk variables for swallowing changes. | eng |

dc.description.resumo | Além do comprometimento pulmonar, doença pulmonar obstrutiva crônica (DPOC)
causa diferentes alterações sistêmicas, dentre elas, a disfunção muscular. Tal
alteração pode ter repercussão também na deglutição, uma vez que a atividade
muscular durante a transferência do bolo alimentar da boca para o esôfago necessita
estar adequada, com o intuito de manter a proteção das vias aéreas. Sendo assim, o
objetivo do presente trabalho foi analisar a relação entre Força de Preensão Manual
(FPM) e Tempo de Trânsito Faríngeo (TTF) em indivíduos com DPOC. Trate-se de
um estudo de caráter quantitativo, transversal e descritivo através da análise de dados
secundários do tipo individual em banco de dados. A amostra foi composta por 16
indivíduos com diagnóstico de DPOC, de ambos os sexos e média de idade de 65,7
anos. Foram coletadas dos prontuários as informações quanto à FPM, assimetria de
FPM, Videofluoroscopia da Deglutição (VFD), protocolo Volume-Viscosity Swallow
Test (V-VST) e Índice de massa corporal (IMC). Os dados receberam tratamento
estatístico por meio do pacote estatístico SPSS (Statistical Package for Social
Sciences), versão 26.0. Foram realizados o teste de correlação de Pearson, o cálculo
do efeito da amostra e regressão logística binária. Observou-se que a maioria dos
indivíduos apresentaram DPOC grave (62,5%), FPM reduzida e o IMC proporcional
entre as classificações (desnutrição, eutrofia, obesidade). Constatou-se ainda, que a
maioria dos indivíduos apresentaram deglutição normal (43,75%), V-VST sem
alterações (62,5%) e TTF para líquido de 0,88 segundos e para pastoso de 0,81
segundos. Quanto às correlações entre as variáveis, não se observou correlação
significativa entre a classificação da deglutição e o restante das variáveis. No entanto,
obteve-se correlação significativa (p = 0,012), mesmo que fraca (r = -0,608), entre TTF
e FPM não dominante (FPM-ND). Com o objetivo de verificar a presença de risco para
deglutição, foi considerado para regressão a variável dependente o V-VST e as
variáveis independentes FPM-D, FPM-ND e assimetria de FPM. Porém, as variáveis
não foram significativas para o modelo de predição. Este estudo demonstrou que
indivíduos com DPOC apresentam TTF mais lento e correlação negativa fraca somente entre as variáveis TTF e FPM-ND. Ainda, não possível considerar as
variáveis FPM e assimetria de FPM como variáveis de risco para alteração de
deglutição. | por |
