Caracterização e qualidade de vida de idosos em uma unidade de diálise renal: desafios para a gestão pública em saúde

Objetivo: Caracterizar a qualidade de vida dos idosos de uma unidade de diálise renal, apontando os desafios para gestão pública de saúde. Métodos: A pesquisa é quantitativa, analítica, descritiva e transversal, a população envolvida é idosa com mais de 60 anos do sexo feminino ou masculino que realizaram hemodiálise no período de janeiro a março de 2010 na referida clinica de nefrologia. As variáveis analisadas foram: sócio demográfica (idade, sexo, estado civil e escolaridade); complicações durante a hemodiálise (cãimbra, hipertensão arterial, fraqueza, hipotensão arterial, perda de peso, coceira, dor de cabeça, anemia, dor, constipação intestinal, arritmia cardíaca, ganho de peso, infecções repetitivas); hábitos de vida (atividade física e de lazer) e a média da qualidade de vida (QV) (funcionamento físico, função física, dor, saúde geral, bem-estar emocional, função emocional, função social, energia/fadiga, lista de problemas/sintomas, efeitos da doença renal, sobrecarga da doença renal, situação de trabalho, função cognitiva, qualidade da interação social, função sexual, sono, suporte social, estimulo da equipe de diálise) avaliado com o instrumento Kidney Disease and Quality of Live-Short Form (KDQOL-SFTM). Resultados: Participaram da pesquisa 10 idosos, sendo que a maioria dos idosos em hemodiálise são homens, com idade predominante entre 60 a 69 anos, casados, com ensino fundamental incompleto e aposentados, cuja renda é proveniente da aposentadoria. O tempo de hemodiálise variou de 1 a 16 anos, os principais eventos relatados foram câimbra e hipertensão arterial, a metade faz exercícios físicos, 20% recebem ajuda para as atividades de vida diária e 40% fazem atividades de lazer. A aplicação do instrumento KDQOL-SFTM, traz a média e mediana com valores baixos no qual comprometem a QV, a dimensão do estimulo por parte da equipe de diálise da função cognitiva, da função emocional, da lista de problemas e sintomas e a qualidade da interação social são dimensões que os pacientes estão mais satisfeitos. Conclusão: Pelo perfil dos idosos do estudo que são casados, de baixa renda e escolaridade entende-se que a gestão pública de saúde tem um grande desafio com o envelhecimento, dentre eles, capacitar profissionais com excelência, gerir políticas inclusivas, dando ênfase ao idoso, oferecendo um suporte adequado para se obter uma melhor QV.
