Influência da força muscular inspiratória na capacidade funcional e qualidade de vida de pacientes submetidos à revascularização do miocárdio
Abstract
A cirurgia de revascularização do miocárdio (CRM) é um procedimento complexo que acarreta
complicações clínicas e funcionais, especialmente sobre o sistema pulmonar. Dentre estas,
destaca-se a redução da força muscular inspiratória que persiste mesmo tardiamente e têm sido
sugerida como determinante da capacidade funcional. O objetivo desse estudo foi verificar se a
força muscular inspiratória exerce influência na capacidade funcional e qualidade de vida (QV)
de pacientes pós CRM inseridos na reabilitação cardíaca (RC) - Fase II. Trata-se de um estudo
prospectivo, quase-experimental, envolvendo 24 pacientes pós CRM submetidos ao exercício
aeróbico, resistido e treinamento muscular inspiratório (TMI), durante 12 semanas, 2 vezes por
semana. Pré e pós RC foram realizadas as seguintes avaliações: força muscular inspiratória
(pressão inspiratória máxima - PImáx) através de manovacuometria; capacidade funcional
máxima (consumo máximo de oxigênio no pico do exercício - VO2 pico) pelo teste
cardiopulmonar de exercício, capacidade funcional submáxima por meio do teste de caminhada
de seis minutos (TC6M) e QV pelo questionário Minnesota Living with Heart Failure
Questionnaire (MLHFQ). Pré programa de RC, a PImáx apresentou correlação positiva
moderada com o VO2 pico (r=0,678; p=0,003) e com o TC6M (r=0,439; p=0,031). Entretanto,
não foi observada correlação com a QV (r=0,015; p=0,942). Houve correlação positiva forte
entre PImáx e VO2 pico (r=0,753; p<0,001) e a PImáx correlacionou-se positiva e
moderadamente com o TC6M (r=0,520; p=0,009) e negativa e moderadamente com a QV
(r=0,456; p=0,025) após o programa de RC. O presente estudo demonstrou que a força muscular
inspiratória é um determinante da capacidade funcional e da QV em pacientes pós CRM
inseridos na Fase II da RC.
