Efeitos de intervenções da fisioterapia com fotobioestimulação e microcorrente na cicatrização e qualidade de vida de indivíduos com úlceras cutâneas: terapias isoladas e associadas
Abstract
As úlceras cutâneas configuram-se como um problema de saúde pública pelo alto risco de infecções e custos com seus tratamentos, bem como pelas internações prolongadas e recorrentes, além das incapacitações físicas, sociais e laborais. Essas feridas podem resultar em limitações funcionais que comprometem a produtividade e a qualidade de vida dos indivíduos acometidos, e até mesmo de seus familiares e acompanhantes. Diante da problemática que as úlceras cutâneas trazem à saúde, terapias que venham a auxiliar essas pessoas são de grande valia. Alguns recursos eletrotermofototerapêuticos utilizados na fisioterapia são apontados como uma forma de tratamento. Neste cenário, este estudo tem o objetivo de investigar o efeito do laser de baixa potência e da microcorrente, isolados e associadas, no reparo cicatricial bem como na qualidade de vida de indivíduos com úlceras cutâneas de diferentes etiologias. Estudo quase-experimental baseado na aplicação de dois recursos eletrotermofototerapêuticos, constituído por uma amostra de 9 pacientes que estavam em atendimento para realização de curativos no Hospital Universitário de Santa Maria (Ambulatório B da Enfermagem) devido a presença de lesões abertas, ocorrida nos últimos cinco anos. Para execução da pesquisa os pacientes foram alocados em três grupos de forma aleatória: Grupo Laser (GL, n=3): tratados com laser de baixa potência vermelho; grupo Microcorrente (GM, n=3): tratados com microcorrente; grupo Laser e Microcorrente (GLM, n=3): tratados com laser de baixa potência vermelho associado a microcorrente. Todos foram submetidos a uma coleta de dados e avaliações da sensibilidade tátil (estesiômetro), nível de dor pela Escala Visual Analógica (EVA) e qualidade de vida utilizando-se o questionário WHOQOL-bref, além de registro com régua e fotogrametria para mensuração da úlcera em centímetros seguido de aplicação do laser ou microcorrente. Aplicado o protocolo de fotobioestimulação ou corrente elétrica ou ambos os pacientes foram reavaliados pelos mesmos instrumentos na 10ª e 15ª intervenção. Pode ser constatado uma redução significativa no comprimento da úlcera cutânea, bem como na largura na 15ª intervenção. Nesse contexto, a área da ferida apresentou uma significante redução na 15ª intervenção. Além disso, houve melhora na qualidade de vida dos pacientes já a partir da 10ª sessão (p<0,05). Conclui-se que o uso da fotobioestimulação e da microcorrente aplicados de forma isolada e associada otimizaram o reparo tecidual das úlceras cutâneas de diversas etiologias promovendo uma redução na área da ferida com consequente melhora na cicatrização e na qualidade de vida dos pacientes. Juntos esses achados contribuem para aprimorar a terapêutica em benefício da qualidade de vida dos indivíduos com úlceras cutâneas.
