dc.description.abstract | In intensive care units, elderly people are exposed to noise, social isolation, physical restriction,
sleep deprivation, invasive devices, often triggering a disorder called delirium. It is an organic
brain syndrome, with acute manifestation and simultaneous presence of disturbances in
consciousness, attention, perception, thinking, memory, psychomotor behavior, emotions,
sleep-wake rhythm, disorientation and disturbances in language, with a prevalence of up to 70%
of the patients analyzed in the ICU. Theoretical models and nursing theories, by supporting care
practices, can contribute to the improvement of theoretical and practical skills in nursing care,
including the prevention of delirium in elderly people in the ICU. For the foundation of nursing
care, Roy's Adaptation Model recognized the possibility of envisioning the integration of theory
with clinical practice for the prevention of delirium in elderly people in the ICU. The research
question that guided this study was: what theoretical relationships can be established between
Roy's Adaptation Model and nursing care in the prevention of delirium in elderly people in the
ICU through a nursing microtheory? And as a general objective: to develop a microtheory, of
the prescriptive type, for nursing care in the prevention of delirium in elderly people in the ICU,
supported by constructs of the Adaptation Model of Callista Roy. A basic, theoretical,
prescriptive research was carried out, with a methodological structure based on the theoretical
substruction process. Roy's Adaptation Model allowed the development of the theory's
deductive path. In order to understand the phenomenon of nursing care in the prevention of
delirium in elderly people in the ICU, an integrative literature review was carried out, as a
method of synthesis, which contributed to the inductive path of the theory. The stages of
conceptualization and operationalization of the microtheory were carried out. In the theoretical
system, Roy's focal stimulus and contextual stimulus constructs were used. Thus, the concepts
of focal care and contextual care were produced and from these subsumed, the adaptive
response to prevention, being considered a variable. In establishing the relational statements of
the theoretical system, four axioms, two postulates, eight propositions and an epistemic
assumption were elaborated. Two empirical indicators were established in the operating system:
the Confusion Assessment Method for Intensive Care Units and the demographic/clinical
history of the elderly (alcoholism, smoking, previous cognitive impairment and dementia). In
establishing the relational statements of the operating system, two transformational statements
and four hypotheses were produced. A flowchart and a protocol were created for the practical
implementation of the microtheory and to guide its use in the stages of the nursing process. This
thesis contributes in an original way by defining nursing care in the prevention of delirium in
the elderly through a construct of interest to nursing, in the organization of professional care
actions in the Intensive Care Unit. | eng |

dc.description.resumo | Nas unidades de terapia intensiva as pessoas idosas são expostas a ruídos, isolamento social,
restrição física, privação do sono, dispositivos invasivos, desencadeando com frequência, um
distúrbio denominado delirium. Trata-se de uma síndrome cerebral orgânica, com manifestação
aguda e presença simultânea de perturbações da consciência, da atenção, da percepção, do
pensamento, da memória, do comportamento psicomotor, das emoções, do ritmo sono-vigília,
de desorientação e perturbações na linguagem, com prevalência de até 70% dos pacientes
analisados em UTI. Os modelos teóricos e teorias de enfermagem, ao sustentar as práticas de
cuidados, podem contribuir para o aprimoramento das habilidades teórico-práticas no cuidado
de enfermagem incluindo na prevenção do delirium em pessoas idosas na UTI. Para a
fundamentação do cuidado de enfermagem, reconheceu-se no Modelo de Adaptação de Roy a
possibilidade de visionar a integração da teoria com a prática clínica para a prevenção do
delirium nas pessoas idosas na UTI. A questão de pesquisa que guiou o presente estudo foi:
quais as relações teóricas podem ser estabelecidas entre o Modelo de Adaptação de Roy e o
cuidado de enfermagem na prevenção do delirium em pessoas idosas na UTI por meio de uma
microteoria de enfermagem? E como objetivo geral: desenvolver uma microteoria, do tipo
prescritiva, para cuidados de enfermagem na prevenção do delirium em pessoas idosas na UTI,
sustentada em construtos do Modelo de Adaptação de Callista Roy. Realizou-se uma pesquisa
básica, teórica, prescritiva, com estrutura metodológica baseada no processo de substrução
teórica. O Modelo de Adaptação de Roy permitiu o desenvolvimento do caminho dedutivo da
teoria. Para conhecer o fenômeno dos cuidados de enfermagem na prevenção do delirium em
pessoas idosas na UTI, realizou-se, como um método de síntese, uma revisão integrativa da
literatura que contribuiu para o caminho indutivo da teoria. Foram realizadas as etapas de
conceitualização e de operacionalização da microteoria. No sistema teórico utilizaram-se os
construtos estímulo focal e estímulo contextual de Roy. Assim, foram produzidos os conceitos
de cuidado focal e cuidado contextual e destes subsumidos, a resposta adaptativa à prevenção,
sendo considerada uma variável. No estabelecimento das declarações relacionais do sistema
teórico elaboraram-se quatro axiomas, dois postulados, oito proposições e um pressuposto
epistêmico. No sistema operacional estabeleceram-se dois indicadores empíricos: o Confusion
Assessment Method for Intensive Care Units e o histórico demográfico/clínico das pessoas
idosas (alcoolismo, tabagismo, comprometimento cognitivo prévio e demências). No
estabelecimento das declarações relacionais do sistema operacional produziram-se duas
declarações transformacionais e quatro hipóteses. Um fluxograma e um protocolo foram
criados para a implementação prática da microteoria e para orientar a sua utilização nas etapas
do processo de enfermagem. A presente tese contribui de forma original ao definir, cuidados de
enfermagem na prevenção do delirium em pessoas idosas por um construto de interesse para a
enfermagem, na organização de ações do cuidado profissional na Unidade de Terapia Intensiva. | por |
