dc.description.abstract | This cross-sectional study aimed to assess the association between self-reported gingival
bleeding, social determinants of health and oral health-related quality of life in a population of
Brazilian adults and elderly. For this purpose, the ELSI-Brasil database – Longitudinal Study
of the Health of the Elderly in Brazil – was used. Self-reported gengival bleeding was measured
using the question "Do your gum currently bleed?" with options of answers dichotomized in
“yes” or “no”. The quality of life variable was verified by the Oral Impacts on Daily
Performance as a categorical variable, dichotomized into “no impact”, for those who answered
no to all questions and “has an impact on their oral health-related quality of life”, for those who
answered yes to at least one question. The analysis of the social determinants of health was
based on the model suggested by Peres and contributors in 2019, formed by three large blocks
of determinants. Sex, education, skin color, age, wealth index, depression, life satisfaction, trust
in the neighborhood, use of toothbrush and dental floss, smoking, dental services, diabetes and
number of teeth were analyzed. At first, the association between self-reported gingival bleeding
as an outcome and associated factors was evaluated. Next, the relationship between oral healthrelated
quality of life as an outcome and self-reported gingival bleeding exposure was sought.
Of the 6,114 individuals included in this study, the prevalence of self-reported gingival bleeding
was 10,4%. There was an independent association with self-reported gingival bleeding and
psychosocial variables such as depression, dissatisfaction with life, lack of trust in neighbors
and smoking. It is also observed that there was a lower perception of gingival bleeding in the
olders. Individuals aged 60-69 years, 70-79 years and over 80 years had, respectively, 27%,
55% and 71% lower prevalence of reporting gingival bleeding than those between 50-59 years
old. Regarding oral health-related quality of life, the prevalence was 46.3%. Those who
reported experiencing gingival bleeding had a higher prevalence of poor oral health-related
quality of life (Reason for Prevalence: 1.10; 95% Confidence Interval: 1.02-1.18) than their
peers, even after adjustment for all variables in the model. Therefore, psychosocial
characteristics were strong associated with self-reported gingival bleeding. And individuals
who report gingival bleeding had a poor quality of life than those who do not report gengival
bleeding. The importance of public health actions that address the community, seeking to
modify health inequities, is highlighted here. | eng |

dc.description.resumo | Este estudo transversal teve por objetivo avaliar a associação entre sangramento gengival
autorreportado, determinantes sociais de saúde e qualidade de vida relacionada à saúde bucal
em uma população de adultos e idosos brasileiros. Para tal, utilizou-se a base de dados do ELSIBrasil
– Estudo Longitudinal da Saúde dos Idosos do Brasil. O sangramento gengival
autorreportado foi aferido através da pergunta “A gengiva do(a) Sr(a) sangra atualmente?”.
Com opções de respostas dicotomizadas em “sim” ou “não”. A variável qualidade de vida
relacionada à saúde bucal foi verificada pelo Oral Impacts on Daily Performance, sendo que
um dos nove itens (sobre prática de esportes) não foi incluído. O Oral Impacts on Daily
Performance foi analisado como uma variável categórica, dicotomizada em “nenhum impacto”,
para aqueles que responderam não em todas as questões e “possui impacto na sua qualidade de
vida relacionada à saúde bucal”, para aqueles que responderam sim para pelo menos uma
questão. A análise dos determinantes sociais de saúde foi baseada no modelo sugerido por Peres
e colaboradores em 2019, formado por três grandes blocos de determinantes. Foram analisados
sexo, educação, cor da pele, idade, índice de riqueza, depressão, satisfação com a vida,
confiança na vizinhança, uso de escova de dentes e fio dental, hábito de fumar, serviços
odontológicos, diabetes e número de dentes remanescentes. Em um primeiro momento foi
avaliada a associação entre sangramento gengival autorreportado como desfecho e os fatores
associados. A seguir buscou-se a relação entre qualidade de vida relacionada à saúde bucal
como desfecho e sangramento gengival autorreportado como exposição. Dos 6.114 indivíduos
incluídos neste estudo, a prevalência de sangramento gengival autorreportado foi de 10,4%.
Observou-se a associação independente entre sangramento gengival autorreportado e variáveis
psicossociais, como depressão, insatisfação coma vida, falta de confiança nos vizinhos. Em
relação à idade, observa-se que quanto maior a idade, menor a percepção de sangramento
gengival. Indivíduos na faixa etária de 60-69 anos, 70-79 anos e mais de 80 anos de idade
tiveram prevalência, respectivamente, 27%, 55% e 71% menor de relatar sangramento gengival
do que aqueles entre 50-59 anos. Também foi observado que aqueles que referiram apresentar
sangramento gengival tiveram maior prevalência de pior qualidade de vida relacionada à saúde
bucal (Razão de Prevalência: 1.10; Intervalo de Confiança 95%: 1.02- 1.18), mesmo após os
ajustes por todas as variáveis do modelo. Portanto, determinantes psicossociais influenciaram
independentemente na autopercepção de sangramento gengival, e este na qualidade de vida
relacionada à saúde bucal. Ressalta-se aqui a importância de ações de saúde pública que
incluam a abosdagem de fatores psicossociais a nível de coletividade buscando modificar as
iniquidades em saúde. | por |
