dc.description.abstract | Acute kidney injury (AKI) and chronic kidney disease (CKD) are diseases which affect the
urinary tract characterized by the loss of renal function. Even having different characteristics,
they can present in similar ways in moments of crisis. However, the therapy for AKI usually
aims the complete renal regeneration, as well as the management of CKD looks for slowing
down their progression and give quality of life to the patient. Mesenchymal stem cells (MSC)
transplantation has spread over the years as a treatment for many diseases. In the urinary tract,
studies involving animal models report anti-inflammatory, antiapoptotic, antifibrotic,
antioxidant and angiogenic effects, slowing down the progression of CKD and accelerating
the kidney regeneration in AKI situation. Although there are studies in dogs and cats
involving the MSC therapy in the urinary tract affections, no systematic reviews with metaanalysis have been conducted until this moment. Thereby, this work reports the results of a
meta-analysis regarding the effects of the MSC application in serum levels of creatinine in
dogs and cats with AKI and CKD. The work followed the methodologic guidelines entitled
“Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA)”. The
bibliographic search involved the following search mechanisms: PubMed, Embase, Cinahl,
Cochrane, Scopus and Web of Science and used three sets of terms: stem cells, kidney injury,
and the species involved in the research. Data were screened, selected, and extracted for a
spreadsheet with characteristics about the studies. The degree of injures were classified
according to their identification and the risk of bias was calculated by the system Systematic
Review Centre for Laboratory Animal Experimentation (SYRCLE). Data underwent a
descriptive analysis. The results of each group were combined by the inverse variance
method. The representation happened as a forest plot. The heterogeneity was evaluated by the

test. For the mean of creatinine, a meta-analysis was performed according to the study
group and number of transplants, and separately for the control and treatment groups
according to the kind of injury, dose, application route, and moment. We found, 4742 articles.
Of these, 40 were selected for eligibility evaluation, 16 of which were sent to qualitative
analysis and nine to the quantitative. The obtained results denote a better outcome in dogs and
cats treated with mesenchymal stem cells over those which received placebo. A statistical
difference could be observed both in combined analysis and in the subgroups division, when
stratifications regarding the kind of injury, route of administration, dose, and measurement
moment. However, a high heterogeneity was found, which indicates considerable variation in
the results between the studies. The reasons can be due to differences in protocols of
treatment, patient characteristics, or in the methodological quality. The great variation
between the studies suggest caution to interpret the results and does not allow a definitive
conclusion. In addition, this work presents two articles accepted for publication which involve
results regarding the bone marrow mononuclear cells transplantation in two dogs, one with
immune mediated thrombocytopenia and a second with chronic hepatopathy. | eng |

dc.description.resumo | A injúria renal aguda (IRA) e a doença renal crônica (DRC) são afecções que afetam o trato urinário caracterizadas pela perda de função renal. Apesar de terem características diferentes, podem se apresentar de forma similar em momentos de crise. Contudo, a terapia para a IRA usualmente visa a completa regeneração renal, enquanto o manejo da DRC objetiva retardar a sua progressão e conferir qualidade de vida ao paciente. Já o transplante de células-tronco mesenquimais (CTM) tem se difundido ao longo dos anos como tratamento para inúmeras enfermidades. No trato urinário, estudos envolvendo modelos animais relatam efeitos anti-inflamatórios, antiapoptóticos, antifibróticos, antioxidantes e angiogênicos, com o retardo da progressão da DRC e a aceleração da regeneração renal em situações em que há IRA. Apesar de haver estudos em cães e gatos envolvendo a terapia com CTM nas afecções de trato urinário, não se conduziu nenhuma revisão sistemática com meta-análise sobre o assunto até o momento. Desta forma, este trabalho relata os resultados de uma meta-análise sobre os efeitos da aplicação de CTM na concentração sérica de creatinina em cães e gatos com IRA e DRC. O trabalho seguiu diretrizes metodológicas intituladas “Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA)”. A busca bibliográfica envolveu os mecanismos de busca: PubMed, Embase, Cinahl, Cochrane, Scopus e Web of Science e utilizou três conjuntos de termos: células-tronco, injúria renal e as espécies que envolviam a pesquisa. Os dados foram triados, selecionados e extraídos para planilha com características sobre os estudos. Classificaram-se os tipos de injúria conforme sua identificação e calculou-se o risco de viés pelo sistema Systematic Review Centre for Laboratory Animal Experimentation (SYRCLE). Os dados passaram por análise descritiva. Combinaram-se os resultados de cada grupo pelo método da variância inversa. A representação aconteceu na forma de forest plot. Avaliou-se a heterogeneidade pelo teste de I2. Para a média de creatinina, realizou-se meta-análise segundo o grupo de estudo e número de aplicações e separadamente para os grupos controle e tratado de acordo com o tipo de lesão, dose, via de aplicação e momento. Ao todo, 4742 artigos foram encontrados. Destes, selecionou-se 40 para avaliação de elegibilidade, 16 dos quais foram para a análise qualitativa e nove para a quantitativa. Os resultados encontrados denotam vantagem ao grupo de cães e gatos tratados com células-tronco mesenquimais em detrimento daqueles que receberam placebo. Pode-se observar diferença estatística tanto na análise combinada quanto na divisão por subgrupos, quando houve estratificações quanto ao tipo de lesão, via de administração, dose e momento da aferição. No entanto, encontrou-se elevada heterogeneidade, que indica considerável variação nos resultados entre os estudos. As razões podem ser diferenças nos protocolos de tratamento, nas características dos pacientes ou na qualidade metodológica. A grande variação entre os estudos indica cautela ao generalizar os achados e torna a conclusão menos definitiva. Esta tese ainda apresenta dois artigos aceitos para publicação que envolvem resultados do transplante de células mononucleares da medula óssea em dois cães, um com trombocitopenia imunomediada e outro com hepatopatia crônica. | por |
